DIELECTRIC VOLTAGE WITHSTAND TEST, UL514D, Para. 5.2.3
UL Requirement - 4.3.2.3.1 - A molded material used for a cover plate shall withstand for 1 minute without breakdown the application of a 60 Hz, sinusoidal potential of 2500 V ac.
Exception No. 1: A cover plate molded of a material that has been found to withstand a 60 Hz, sinusoidal potential of at least 2500 V ac is not required to be tested.
Exception No. 2: A cover plate of phenolic or urea composition with a minimum thickness of 2.54 mm (0.1 in) is not required to be tested.
UL Test Method - 5.2.3.1 - Six cover plates shall be subjected to the application of a 60 Hz, sinusoidal potential of 2500 volts ac across the thinnest section of the cover plate. Each sample shall then be conditioned for 24 h in moist air having a relative humidity of 85 ±5 percent at a temperature of 32 ±2°C (90 ±4°F). Following this conditioning, each sample shall again be subjected to the specified test potential.
FLAME PENETRATION TEST (5VA Test), UL514D, Para. 5.2.4
UL Requirement - 4.3.2.4.1 - When subjected to the flame penetration test in accordance with Clause 5.2.4:
a) a cover plate shall not openly flame for more than 1 minute after the fifth application of the test flame;
b) there shall not be visible flame on the surface of the cover plate opposite the surface to which the test flame is applied; and
c) there shall not be an opening through the cover plate after the test.
Exception: A material designated 5VA is not required to be tested.
UL Test Method - 5.2.4.1 - Six cover plates shall be tested. Three shall be tested as-received and three shall be tested after being conditioned for 168 h at a temperature of 90°C (194°F) in an air-circulating oven. Each sample shall be subjected to the test described in Clause 5.2.5.2 – 5.2.5.6, except that the valve supplying the gas shall be opened for 5 s and closed for 5 s in each of the five applications of the flame.
5.2.5.2 The test shall be conducted in a three-sided enclosure that is 305 mm (12 in) wide, 356 mm (14 in) deep, and 610 mm (24 in) high. The top and front of the enclosure shall be open. The room or hood in which the enclosure is located for the test shall be ventilated, but drafts shall be prevented from affecting the test flame.
5.2.5.3 A Tirrill gas burner to which a gas pilot light is attached shall supply the test flame. The barrel of the burner shall extend 102 mm (4 in) above the air inlets, and its inside diameter shall be 9.5 mm (3/8 in). The gas supplied for the flame shall produce 30.198 ±0.296 kJ/m3 (1010 ±10 Btu/ft3). With the barrel vertical, the overall height of the flame shall be adjusted to 127 mm (5 in). The blue inner cone shall be 38.1 mm (1-1/2 in) high. Without disturbing the adjustments for the height of the flame, the valves supplying gas to the burner and pilot flames shall be closed.
5.2.5.4 A wedge to which the base of the burner can be secured shall be provided for tilting the barrel 20° from the vertical while the longitudinal axis of the barrel remains in a vertical plane. The burner shall be secured to the wedge and the assembly shall be placed in an adjustable jig that is attached to the floor of the enclosure. The jig shall be adjusted laterally (see Figure 5) to place the longitudinal axis of the barrel in the same vertical plane as the vertical axis of the specimen. The plane shall be parallel to the sides of the enclosure.
5.2.5.5 The jig shall be adjusted toward the rear or front of the enclosure (see Figure 5) to position point A, which is the intersection of the longitudinal axis of the barrel with the plane of the tip of the barrel
38.1 mm (1-1/2 in) from the point B, at which the extended longitudinal axis of the barrel meets the front surface of the specimen. Point B is the point at which the tip of the inner blue cone will touch the specimen. The specimen shall be adjusted vertically to place point B at the center of the specimen. The specimen is to be located so that the test flame is directed at the outside of the cover plate and does not impinge on any edge or opening of the cover plate.
5.2.5.6 The valve supplying gas to the pilot shall be opened and the pilot flame lit. The valve supplying gas to the burner shall be opened to apply the flame to the specimen automatically. This valve shall be held open for
15 s and then closed for 15 s. The procedure shall be repeated four times for a total of five applications of flame to the specimen.
FLAMMABILITY TEST, UL514D, Para. 5.2.5
UL Requirement - 4.3.2.5.1 - A cover plate or outlet box hood shall not openly flame for more than 1 min after the fifth application of a standard test flame. Penetration of the flame through the specimen is allowed.
Exception No. 1: A material designated 5VA or 5VB is not required to be tested.
Exception No. 2: A removable appendage – hinged cover, boot, or similar part – that is designated HB and intended solely to restrict the exposure of a device opening to moisture is not required to be tested.
UL Test Method - 5.2.5.1 - Six specimens shall be tested. Each specimen shall be one of the following:
a) a square plaque having minimum dimensions 102 x 102 mm (4 x 4 in) and a thickness not exceeding the minimum thickness of the cover plate;
b) a circular plaque having a minimum diameter of 114 mm (4.5 in) and a thickness not exceeding the minimum thickness of the cover plate; or
c) a finished blank cover plate.

AGING TEST, UL514D, Para. 5.2.6
UL Requirement - 4.3.2.6.1 - There shall not be evidence of cracking of a cover plate after the conditioning.
Exception: A cover plate of phenolic or urea composition with a minimum thickness of 2.54 mm (0.1 in) is not required to be tested.
UL Test Method - 5.2.6.1 - Six samples of a cover plate shall be exposed for 168 h at a temperature of 90 ±1°C (194±2°F) in an air-circulating oven.
MECHANICAL STRENGTH TEST, UL514D, Para. 5.2.7
UL Requirement - 4.3.2.7.1 - A cover plate shall not crack while subjected to the compressive force and shall not be permanently deformed such that the center of the plate is displaced more than 1.6 mm (0.0625 in) from its initial position.
Exception: A cover plate of phenolic or urea composition with a minimum thickness of 2.54 mm (0.1 in) is not required to be tested.
UL Test Method - 5.2.7.1 - Samples used for the mechanical strength test:
a) shall be aged in accordance with Clause 5.2.6and shall be conditioned for 168 h at 50 percent relative humidity at 23 ±2°C (73 ±4°F) prior to the application of the compressive force, or
b) shall be untested samples conditioned for 168 h at a minimum of 60°C
(140°F) prior to the application of the compressive force.

5.2.7.2 Each sample shall be placed face up and centrally located on two parallel 6.35 mm (0.250 in) diameter steel rods of such length that they extend beyond the extremities of the plate, and are spaced 60 mm (2.375 in) apart with the short edge of the plate parallel to the rods. A third, 6.35 mm diameter rod of similar length shall be placed on the face of the plate midway between and parallel to the other rods. The assembly shall be placed in a compression machine, the jaws of which close at the rate of 12.7 mm/min (0.50 in per min) (see Figure 1). The compressive force shall be applied until one of the following is obtained:
a) a displacement of 6.35 mm (0.25 in); or
b) a force of 111 N (25 lb)
The test shall be repeated with the long edges parallel to the rods.
5.2.7.3 The displacement of the cover plate from its original position shall be measured when the force is removed.
MOLD STRESS RELIEF TEST, UL514D, Para. 5.2.8
UL Requirement - 4.3.2.8.1 - A cover plate shall not show evidence of distortion by an amount greater than or equal to 1.6 mm (0.0625 in).
Exception: A cover plate of phenolic or urea composition with a minimum thickness of 2.54 mm (0.1 in) is not required to be tested.
UL Test Method - 5.2.8.1 - Six samples of a cover plate shall be exposed for 7 h to a temperature of 90 ±1°C (194 ±2°F) in an air-circulating oven.
